A starting point for a conversation.

This is self-report screening and care navigation, not a clinical service. Results do not confirm or rule out a diagnosis, establish a disease probability, or recommend medication. The combined assessment has not itself undergone clinical validation or clinician review. Each established instrument has its own evidence, timeframe and limitations; presenting them together does not create a validated overall test.

Symptoms can overlap and reflect physical illness, medication, substances, sleep or circumstances. Screening performance varies across populations and settings. Seek care for concerns even with a low score. No combined score is calculated, and this assessment does not cover every condition.

How the five scores work.

Screen / timeframeScore and interpretation
PHQ-9 · Last 2 weeksNine responses scored 0–3, total 0–27. Bands: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe, 20–27 severe. A total of 10 or more prompts assessment. Any nonzero response to item 9 displays safety guidance regardless of total; that response alone does not establish immediate risk.
GAD-7 · Last 2 weeksSeven responses scored 0–3, total 0–21. Bands: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–21 severe. A total of 10 or more prompts assessment. This does not assess all anxiety disorders.
ASRS v1.1 six-question screener · Past 6 monthsCount responses meeting the original item-specific rule: Sometimes or more for items 1–3; Often or more for items 4–6. Four or more of six prompts further assessment. This is not a severity score or diagnostic probability.
PC-PTSD-5 · Past monthAfter the lifetime trauma gate, count Yes responses to five items (0–5); the gate is excluded. No at the gate stops the screen with zero. This page uses the VA public self-screen threshold of 3. Some clinical settings use 4; performance varies by population and purpose.
AUDIT-C · Past yearThree items scored 0–4, total 0–12. Never drinking stops the screen at zero. Item 3 preserves the six-or-more-drinks version. Common reference cutoffs: 3 for women, 4 for men. You may choose either reference or score only; no sex or gender is inferred. Binary reference groups may not fit everyone. If all points come from frequency alone, discuss actual intake with a clinician.

Unanswered required items stay incomplete; they are never assigned zero. The optional daily-function question is separate from the totals. Skipping a question or section is allowed.

Wider reflections are unscored.

Nine original prompts organize concerns about mood highs, perception, obsessions and compulsions, eating, panic, sleep, other substances, autism, and stress or grief. They are not validated diagnostic tests. Yes and Unsure identify conversation topics, not conditions you have. Psychosis does not establish schizophrenia. Autism is neurodevelopmental, not a mental illness.

The reflections do not embed the licensed MDQ, AQ or AQ-10 and cannot provide validated bipolar, schizophrenia, OCD, eating-disorder or autism scores. Mood and perception concerns display prompt-help guidance before completion.

Your answers stay in this browser.

No account is required. Answers and scores are held in memory in this page. They are not sent to STATIC, an AI service, a provider, an advertising service or another STATIC product. No appointment is booked and nobody is notified. Ordinary page requests can still expose the page URL, IP address and browser information to hosting infrastructure. Local answers do not make browsing anonymous.

Save draft is off by default. Turning it on stores answers, screen selections and the optional AUDIT-C reference choice in this browser’s local storage. Storage is not encrypted or protected by your STATIC account. Anyone with access to this browser profile or other code on this site may be able to read it. Avoid saving on a shared device. Browser settings, extensions and device backups can affect privacy.

Restore saved answers loads a draft without automatically enabling continued saving. Stop saving & remove draft deletes the stored copy while retaining answers in this open page. Delete all answers clears this page and its saved draft after confirmation. Close or refresh other assessment tabs separately; they may still hold answers and save them again. Clearing all site storage can also remove other STATIC preferences.

Download private summary creates a sensitive text file on your device with results, flagged reflections and care links. It is not uploaded. Protect and delete downloaded or shared copies separately. Changing screen selections does not erase earlier answers; use Clear answer or Delete all answers to remove them. If storage is unavailable, you can continue without saving.

Support is available at any score.

Help is available before you finish.

If you may act on thoughts of harming yourself or someone else, cannot stay safe, or have a medical emergency, contact emergency services or go to an emergency department now. In the U.S., call 911. If possible, stay with someone you trust.

U.S. emotional distress or suicide crisis support: call 988, text 988 or chat with the 988 Lifeline. Outside the U.S., use your local emergency or crisis service.

YOUR NEXT CONVERSATION

Find the right kind of support.

Start with primary care if you are unsure. A therapist can assess emotional and behavioral concerns; a psychiatrist can assess complex mood or perception changes and medication needs. For ADHD or autism, ask specifically about adult assessment experience.

What to ask when you contact a provider
  • Do you assess this concern in adults, and what does an assessment involve?
  • What are the fees, insurance options, wait times and lower-cost alternatives?
  • What should I do if symptoms worsen before my appointment?
  • What are your confidentiality rules and accessible appointment options?

You can bring the summary, explain when the changes began, and describe effects on sleep, daily responsibilities and relationships. Include medications, substances and relevant medical history directly with your clinician.
