If you may act on thoughts of harming yourself or someone else, cannot stay safe, or have a medical emergency, contact emergency services or go to an emergency department now. In the U.S., call 911. If possible, stay with someone you trust.

U.S. emotional distress or suicide crisis support: call 988, text 988 or chat with the 988 Lifeline. Outside the U.S., use your local emergency or crisis service.

YOUR CHECK-IN / YOUR CHOICE

Where would you like to begin?

Choose any of five established screens and an optional wider reflection. You can skip questions, leave a section or view a partial summary. Allow about 10–20 minutes for everything; go at your own pace.

Scores describe self-reported symptoms. They cannot confirm or rule out a diagnosis, replace a professional assessment, or cover every mental health condition. No overall “mental health percentage” is calculated.

Answers stay in this open page unless you choose Save draft. No answers are sent to STATIC. The page is not monitored and does not alert anyone. Read privacy details.

Under 18? This version is for adults. A trusted adult, pediatric clinician or school counselor can help you find age-appropriate support. Help contacts remain available below.

YOUR NEXT CONVERSATION

Find the right kind of support.

Start with primary care if you are unsure. A therapist can assess emotional and behavioral concerns; a psychiatrist can assess complex mood or perception changes and medication needs. For ADHD or autism, ask specifically about adult assessment experience.

01 / FIND CARE

Search by your location.

U.S. treatment providers, with service and payment filters. Enter your location on the directory, not in this assessment. Confirm availability, qualifications, cost and insurance directly.

Open FindTreatment.gov ↗
02 / COST & ACCESS

Care on your budget.

Find a U.S. HRSA-supported health center. Ask about mental health services and a sliding-fee assessment; services and eligibility vary.

Find a health center ↗
03 / TALK TO SOMEONE

Help finding treatment.

SAMHSA’s free, confidential U.S. treatment-referral service is available 24/7 in English and Spanish. It provides information and referrals, not counseling.

What to ask when you contact a provider
  • Do you assess this concern in adults, and what does an assessment involve?
  • What are the fees, insurance options, wait times and lower-cost alternatives?
  • What should I do if symptoms worsen before my appointment?
  • What are your confidentiality rules and accessible appointment options?

You can bring the summary, explain when the changes began, and describe effects on sleep, daily responsibilities and relationships. Include medications, substances and relevant medical history directly with your clinician.

Directories and referrals are independent of STATIC. No appointment or endorsement is implied. U.S. contact information checked October 30, 2026. Elsewhere, contact your local health authority or a licensed provider in your country. Sources and methods ↗
